Documents Required (In‑Person Applications)

Please bring: Photo ID (adults only)

• Social Security cards for everyone in the household

• Proof of income: paycheck stub, Social Security/SSI, unemployment income

• Rent or mortgage statement

• Recent utility bill

Eligibility: This program serves households with children age 14 or younger who live in Jackson/Madison County.
